Supported Community Living

Chatham Oaks' SCL program is designed for persons with a diagnosis of chronic mental illness, 18 years of age or older and who are able to live in the community with supportive services.  Our mission is to facilitate self-sufficiency and maximize individual goal attainment in a community setting.  The overall program goal is to provide support to each individual so he or she can function independently in the living, learning, working and socializing environments of their lives.

All services will foster individual choice, rights and self-sufficiency in the completion of activities of daily living and increasing the individual's abilities to make responsible decisions.  All interventions and services will respect and enhance the individual's abilities, encourage a sense of achievement and allow the individual to choose to continue or modify their participation in the treatment process.

Services offered by Chatham Oaks SCL program include but are not limited to:
 

  • Assistance with Medication and Medical Monitoring
  • Physical and Mental Health Monitoring
  • Grocery Shopping, Meal Planning and Preparation
  • Housekeeping and Laundry Support
  • Assistance with Money Management
  • Assistance with Social/Leisure Skill Development
  • Assistance with Problem Solving and Coping Skills
  • 24-Hour Crisis Management

Habilitation Services

Habilitation is a Medicaid service and must be approved by the individual's case manager.  Chatham Oaks offers hourly Home-Based Habilitation Services in the SCL program.

Home-based habilitation consists of individualized services and supports that assist with the acquisition, retention, or improvement in skills related to living in the community.  These services are provided in the individual’s home or community and assist the individual to reside in the most integrated setting appropriate to their needs.  Services are intended to provide for the daily living needs of the individual and can be provided at any time of day or night that is necessary to meet their needs.  This includes the following supports:

  • Adaptive skill development
  • Assistance with activities of daily living
  • Community inclusion
  • Transportation (except to and from a dayprogram)
  • Adult educational supports
  • Social and leisure skill development
  • Personal Care
  • Protective oversight and supervision
