Chatham Oaks provides a continuum of care for individuals with a chronic mental illness needing support services in a residential care facility or in their own apartment or home.  Chatham Oaks provides an opportunity for independent living at the Melrose Ridge Apartments located adjacent to the Chatham Oaks residential care facility on Melrose Avenue in Iowa City.
 
Services offered by Chatham Oaks include Residential Care Facility (RCF), Supported Community Living (SCL) and Home-Based Habilitation.  Services are described in more detail on this website.

A city bus stop is conveniently located outside the door to Chatham Oaks.  Bus service is available hourly from 7:00 a.m. - 6:00 p.m. Monday thru Friday.
 
 
Chatham Oaks is located just 4 miles from the University of Iowa Hospitals and Clinics.  Chatham Oaks' staff take individuals to and from appointments at UIHC and other locations.

 

 

Referrals and Admissions

 

Chatham Oaks will process requests for admission regardless of race, creed, color, religion, age, and/or ethnic/national origin. Chatham Oaks serves adults with a mental illness and/or developmental disabilities. 

 

The following criteria are assessed for each individual referred for admission:

 

  • Age (18 or older)
  • Mental illness/Developmental Disability diagnosis
  • County/State/Private Pay available
  • Not actively suicidal/homicidal
  • Free of self-harm behavior for at least 2 weeks
  • Not on the sex offender list 
  • Criminal background check void of extensive activities such as assault, use of weapons. drug/alcohol related crimes, etc.
  • Substance abuse not a primary problem
  • Wants to/agrees to come to Chatham Oaks and cooperate with the rules & individual responsibilities
  • Mentally/physically stable on the tour or face to face assessment
